by mail, Medical Compliance will call regarding shipping fees according to
weight and destination. Send completed order form with check or money
order to above address. Residents of Michigan must include 6% sales tax.
- PRICING POLICY: All prices are subject to change without notice. You will
be informed of any increase at the time of order.
- SHIPPING: All orders will be shipped within 24 hours, unless there is a
back order. You will be informed of any back order to determine if partial
delivery is acceptable at that time. All C.O.D. orders must be made with
cashier’s check or money order with a $7.00 charge for every C.O.D.
shipment.
- RETURN POLICY: A credit or refund will be issued on products that are
returned undamaged and in the original containers. Special orders cannot
be returned for any reason. Damaged items should be noted with the carrier (i.e., UPS) at time of delivery. All items ordered incorrectly are the
customers responsibility for return charges.
- CREDIT: Credit after the initial order can be given under the following
conditions. You must be an established owner of a business or the owner
must sign a waiver for financial responsibility for any of their employees. If
credit is issued, a monthly finance charge of 1.5% will accrue on any order
after 30 days.
